To register at LPDN:…
- Your child must be 3 years old by September 30 to register for our 3 year-old program.
- Your child must be 4 years old by September 30 to register for our 4 year-old program.
- Your child must be 2 years, 6 months by February 4 to register for our Pre-3’s program.
- Your child must be potty-trained. The only exception to this is the Pre-3’s program.
- Submit a signed application form along with registration fee.
- Submit the required health records: your child’s Universal Child Health Record and the Immunization Record.
- Submit documentation of a Seasonal Flu immunization administered between September 1 and December 31.
